I hereby certify that the design and artwork in this submission is the original creation of the donating artist and is not an infringement of copyright laws of any other publication.

I understand that The Cradle Project is not to be held liable for the damage or loss of any property, and that once the artwork is submitted to The Cradle Project, The Cradle Project becomes sole proprietor of said artwork and thus retains all rights to the piece including but not limited to multi-media display of the project for the promotional purposes of The Cradle Project.

  The Cradle Project requests that all original pieces be submitted by the artist.  The Cradle Project also requests that the donating artist find sponsorship for their piece in the amount of $100.  This small amount is crucial to the success of The Cradle Project in its ability to promote awareness and raise financial support to help feed, shelter, and educate children orphaned by HIV / AIDS in Sub-Saharan Africa.  The New Mexico Community Foundation is providing fiscal sponsorship for tax-deductible donations.  Please have sponsors make donations to: ‘New Mexico Community Foundation’ with ‘The Cradle Project’ written in the memo section.  Mail donations to The Cradle Project, 1937 Truman NE, Albuquerque, NM 87110.

Please submit artist agreement as early as possible but no later than, April 15th, 2008 to the above address.
